  Managing In-Hospital Anticoagulation: The Who, When and How

  Thrombosis is a significant risk for hospitalized patients, driven by factors such as inflammation, comorbidities, infections and immobility. Conversely, these patients also face heightened bleeding risks due to acute kidney injury, coagulopathy, thrombocytopenia and invasive procedures. This dual threat necessitates individualized, daily assessments to optimize anticoagulation therapy, minimizing complications while enhancing outcomes.
