A case report on the first successful ex utero intrapartum intrapartum (exit) procedure on a pregnancy complicated with cystic hygroma

Abstract :

Cystic hygromas are the cystic variety of lymphangioma, common locations being cervico-facial regions and axilla. Respiratory distress, recurrent infections or cosmetic reasons are the main indications of the treatment. This report reviews the current literature and discusses the various problems encountered during the management of pregnancy complicated with this kind of lesion. This is a case of a 34-year-old, G3P2 (2002), who was initially seen at our institution at 31 5/7 weeks AOG for prenatal check-up. There was an incidental finding on transabdominal ultrasound of a cystic structure arising from the right posterior neck area measuring 8.5 x 8.8 x 6.63 suggestive of a cystic hygroma. Plan was for primary caesarean section with Extra utero Intrapartum Treatment procedure in coordination with the Departments of Anesthesia, Pediatrics and General Surgery. At 37 weeks AOG, patient underwent the contemplated multidisciplinary surgery. The fetal cystic hygroma was currently being managed medically through sclerotherapy with Bleomycin under the Department of Pediatrics. In conclusion, management of pregnancy complicated with cystic hygroma proved the significance of antenatal surveillance. Antenatal care paved the way to anticpate the mode of delivery and even gave an opportunity to solicit the expertise of different departments (Anesthesia, Pediatric Surgery, ENT). Cystic hygroma is a manageable lesion in the pediatric population. Appropriate management shoud be opted based upon a case to case variation. Optimum treatment can be given by surgery alone, sclerotherapy alone or combined use of both just what we applied in our patient.

Biography :

Ma Christina Espeleta has completed her Doctor of Medicine at the age of 26 years from Pamantasan ng Lungsod ng Maynila as a full scholar. Her pre medicine course was Bachelor of Science in Nursing from Makati Medical Center College of Nursing wherein she graduated as the number 1 cum laude. She was also a recipient of Presidnet’s Award and Director of Nurisng Award for Clinical Efficiency. She is both a regstered nurse and doctor. She is currently a military doctor in the Armed Forces of the Philippines, taking up residency in the Department of Obstetrics and Gynecology
