A prospective study on “pterygium excision and conjunctival autograft-without suture, without glue”

Abstract :

Purpose: To determine the outcome of sutureless, glue free conjunctival autograft for pterygium surgery as a safe, effective and economical alternative to other techniques and to study the efficacy and complications of sutureless and glue free conjunctival autograft for the management of primary pterygium. Materials: This was a prospective, case study for six months of the patients presented with primary pterygium. The study was carried out among the patients presented with primary pterygium (both nasal and temporal) in the cornea department of National Institute of Ophthalmology and Hospital, Dhaka from June 2013 to December 2013. The study population was comprised of 47 patients. The study population was chosen by purposive sampling. The age range of the patients was 11 to 60 years. Mean age was 37.63 years. Patients with recurrent pterygium were excluded from the study. Visual acuity at presentation and after operation and clinical details on examination were recorded. Pterygium excision with limbal (superior) conjunctival autograft without using fibrin glue or sutures was performed in all the patients. They were examined for subconjunctival hemorrhage, graft displacement, chemosis, graft dehiscence, graft retraction, recurrence or any other complication. Appropriate statistical method was used to analyze the data. Results: Of total 47 patients, there were 34 (72%) males and 13 (27.66%) females. Majority of the patients had single head (95.74%) and nasal (91.48%) pterygium. After operation, 14 (29.79%) patients had FB sensation, 12 (25.53%) patients had watering. Sixteen patients (34.04%) had no complaint. So majority of the patients (34.04%) had no complaint after operation. Graft dislocation was found in 2 (4.26%) patients, partial graft dehiscence was found in 5 (10.64%) patients, graft retraction was found in 1 (2.13%) patient, transient graft oedema was found in 25 (53.19%) patients, mild subconjunctival hemorrhage found in 11 (23.40%) patients and recurrence was found in only 1 (2.13%) patient. Mean duration of surgery was 22.26 minutes. At 6 weeks postoperatively, 61.70% of the patients achieved normal visual acuity 6/6. But 36.17% of the patients presented with normal visual acuity 6/6 before operation. So visual acuity was improved after operation. Conclusion: Sutureless and glue free conjunctival autografting following pterygium excision is a safe, fast, effective and economical option for the management of primary pterygium. The newer procedure of conjunctival autografting is free of any untoward complication.

Chandra Shekhar Majumder completed Bachelor of Medicine and bachelor of Surgery (MBBS) from Sir Salimullah Medical College, Dhaka in 1988. He also completed Master of Public Health (Health Education) from National Institute of Preventive and Social Medicine (NIPSOM), Dhaka in 1991. Chandra Shekhar Majumder has completed Fellow of College of Physicians and Surgeons (FCPS) in Ophthalmology from Bangladesh College of Physicians and Surgeons (BCPS) in 2000. He got Short Term Fellowship in Cornea from Venu Eye Institute and Research Centre, New Delhi, India in 2011. He has published more than 20 papers in reputed journals and has been serving as the editor of the Bangladesh Journal of Community Ophthalmology
