The Bacillus Calmette-Guerin (BCG) vaccine, which is a live attenuated strain derived from Mycobacterium bovis, is widely administrated for newborn infants in the world for preventing disseminated and meningeal tuberculosis. However, adverse reactions to the BCG vaccine do occur. Here, we present a case of a 2-year-old girl with left axillary lymphadenopathy, the complication of BCG vaccine. Her medical history showed that she has been vaccinated with BCG two days after her birth. The Computed Tomography (CT) scan revealed two irregular coarse calcified masses in her left axilla. She was then treated with surgery followed by antibiotic Isoniazid 150 mg QD (10 mg/kg/day) for 5 weeks and was followed up after 6 months and no recurrence occurred. Although the incidence of BCG vaccination-associated lymphadenopathy is low, pediatricians should be alerted about this to prompt early surgical intervention and appropriate antibiotics treatment.
