Diabetic retinopathy represents one of the leading causes of visual impairment and diabetic eye disease is the fifth most common cause of blindness according to new researches. The number of people with moderate to severe visual loss due to DR is constantly growing. Current treatment modalities include intravitreal injections (anti-VEGF, corticosteroids) and/ or laser photocoagulation for the therapy of diabetic macular edema (DME) and vitreoretinal surgery for proliferative stages of diabetic retinopathy (PDR). Improvement in visual acuity after the intravitreal and/or laser therapy can be limited to some extents either as a result of the late initiation of the therapy, limited response to the therapy or as a result of late complication. In those groups of patients, pars plana vitrectomy can present the only solution for visual improvement. Our study presents the results of combined three port pars plana phaco-vitrectomy with internal limiting membrane (ILM) peeling for epimacular membrane surgery and bimanual four port combined phaco-vitrectomy for advanced proliferative stages of DR. According to our results, combined phaco-vitrectomy procedure presents the treatment of choice for patients with severe visual loss, vitreous hemorrhage combined with epimacular membrane and retinal proliferations. Improvement of postoperative visual acuity was influenced by the condition of the macula before the surgery and previous treatments for diabetic eye disease.
