Annexel pathology (the ovarian one, particularly) represents a frequent cause of consultation in gynecology for the woman in genital activity. Surgery becomes inescapable in 30% of the cases. This surgery, though often treats the adnexal anomaly, can also affect fertility. It is a retrospective study concerning a 2 months period including 20 patients of 16 to 45 years, operated for an ovarian pathology that has been classified as a very low risk of malignancy clinically and biologically detected through ultrasounds and CT scan. 74% of the remaining was operated by coelioscopy that had been converted to laparotomy in 10% of the cases; 26% of the patients were operated from the beginning by laparotomy. The intraperitoneal cystectomy is the most frequently carried out operation with a rate of 69%; 3 of the 17 patients consulted for a post-surgical fertility problem. The average age of the patients was 31.2 years. From these 3 patients, one has not been pregnant before surgery and 1 was operated by laparotomy. The spontaneous fertility rate after surgery was assessed in many studies that met the inclusion criteria. The negative impact of ovarian cyst surgery on ovarian reserve has been previously reported. The data of our study (9%) confirm this rate and insist on the importance of the number of former pregnancies before surgery and the initial surgical technique used.
