Unplanned extubation is the most common adverse event occurring in intensive care units (ICUs) and significantly increases morbidity and mortality in children. Limited data are available regarding unplanned extubation in Ethiopian. Therefore, this study aimed to determine the incidence, outcome and identify the determinants of unplanned exudation among children in the pediatric intensive care unit in Addis Ababa, Ethiopia in 2023. Methods: Nested unmatched case-control study was conducted among intubated children admitted to selected government hospitals in Addis Ababa from September 1, 2022, to April 30, 2023, with a total of 198 patients (66 cases and 132 controls) were included in this study. Cases were children who had unplanned extubation and controls were planned extubation patients after completing the full weaning process or based on the hospital protocols. For both cases and controls, we also collected data associated with UPE events, re-intubation and outcomes by using a properly constructed data-abstraction form, cleaned, entered into Epidata version 4.6, and exported to SPSS version 25.0 software for analysis
