Intra-abdominal Kikuchi disease mimicking Malignant Lymphoma in an adolescent: A case report

Abstract :

Case Description: Kikuchi disease is characterized by necrotizing lymphadenitis with predominant histiocytic infiltration. This disease usually involves cervical lymph nodes and localized involvement of other sites, especially of mesenteric lymph nodes, is unusual. Clinical Findings: A 14-year-old girl was admitted with right lower quadrant abdominal pain and fever. She underwent appendectomy for acute appendicitis 18 months prior. Abdominal computed tomography and positron emission tomographycomputed tomography showed enlarged mesenteric lymph nodes with high uptake. Treatment & Outcome: She underwent laparoscopic biopsy. Microscopic examination showed Kikuchi disease. After laparoscopic biopsy, fever was improved. Three months after discharge, follow-up abdominal sonography showed a decrease in mesenteric lymph node size, compared with that observed on previous CT. Clinical Relevance: We report a case of typical Kikuchi disease with mesenteric lymph node involvement. In the absence of other symptoms and abnormal findings, such as fever, hepatosplenomegaly and abnormal complete blood cell count, an accurate diagnosis is necessary to avoid unnecessary surgery or aggressive management. As in our case, Kikuchi disease should not be ruled out in children presenting with right lower quadrant pain if lymphadenopathy.

Biography :

Kyu seok Shim is a resident of pediatrics, work in college of Medicine at Chung-Ang University of Seoul, Korea. He has completed his Graduation at Seoul, Korea, and Pediatric Residency in Korea (Chung-Ang university hospital). His primary educational and research interest is in pediatric nephrology. He is currently involved with investigating incidence pattern of disease such as HS purpura. In seok Lim is a Professor, Clinical Nephrologist and Lecturer in college of Medicine at Chung-Ang University of Seoul, Korea. He has completed her Graduation at Seoul, Korea, and Pediatric Residency in Korea (Chung Ang university hospital). After fellowship about pediatric nephrology in Seoul national university hospital, his primary research interest is in pediatric nephrology and gastroenterology. He is currently involved with investigating incidence pattern of diseases such as intusucception of HS purpura.
