Left ventricular noncompaction(LVNC) is a cardiomyopathy characterized by prominent left ventricular trabeculae and deep intertrabecular recesses. It is based on the arrest of endomyocardial morphogenesis during embryogenesis and it was first described in 1984 by Engberding and Bender who described it is isolated â??sinusoidsâ?? within the LV. Here two layers of myocardium, a thin compacted layer outside and deep intertrabecular recesses. The 3 morphologic elements of LVNC: trabecular, the compacted layer, and the intertrabecular recesses. The high noncompaction/compaction (NC/C) ratio that defines the diagnostic criteria of LVNC.The 2008 European society of cardiology working group on myocardial and pericardial diseases categorized LVNC as an â??unclassifiedâ? Cardiomyopathy. Its prevelence is about 0.014 to 1.3 and 3-4% in heart failure patients.Itâ??s clinical manifestation is highly variable, from no symptoms to congestive heart failure, arrhythmias, systemic thrombo-embolism causing transient ischaemic attack (TIA), and stroke. Our case, Mr. X, 85 years gentleman, non-diabetic, but hypertensive, smoker feels dyspnoea and chest discomfort on mild to moderate exertion. He had history of recurrent stroke with right sided weakness and slurring of speech for 7 years but able to move and walk with taking some help yet now. ECG shows left ventricular hypertrophy. Chest x-ray shows borderline enlargement of cardiac shadow. Echocardiogram reveals multiple trabeculation and recesses in the left ventricular apex. Non-compaction and compaction ratio of LV apex is also >2/1.
Gobinda Kanti Paul has completed his MD Cardiology, from NICVD under BSMMU, January 2008. He has more than 10 publications in different index journal one of the journals is in our medical college, named MMJ. One of his articles was published in International Journal of Clinical Medicine Research of AASCIT. He is also a Life Member of Indian Academy of Echocardiography. He has passion for nonintervention cardiology specially echocardiography and preventive cardiology. Currently, he is an Assistant Professor/Consultant Cardiology and Residential Physician in the Mymensingh Medical College and Hospital. He has special interest in rheumatic fever and rheumatic heart disease.
