Blisters in neonates is rare and the differential diagnosis is extremely broad, ranging from infectious disease to genetically inherited disorder. Determining the underlying cause of the blister is a challenge, especially in rural area. We reported a case of newborn with blisters, accompanied by infection (both local and systemic) and a history of seizures. A 6 days old neonate was referred to the emergency department and was presented with multiple vesicles, pustules, and bulla on his trunk and extremities. Upon hospitalization, he developed a systemic infection followed with an episode of seizures. Due to limited resources available in our hospital, we use a lactic acid solution to bath him, wound treatment, moisturizer, and antibiotic treatment. Since our hospital is located in the rural area, establishing a confirmed diagnosis and treating the blisters according to the guidelines are proven to be difficult. A referral is needed to do a further assessment of the condition.
