Graft-versus-host disease (GVHD), an immune-mediated disease caused by complex interactions between donor and recipient immune systems. It is a leading cause of morbidity and mortality following hematopoietic stem cell transplantation (HSCT), and its incidence is reported in 1974 to be 70.5%. Even with newer immunosuppressive medications and methodologies, the incidence is still between 25% and 70% today. Ocular manifestation can range from mild bothersome symptoms to intractable visual impairment affecting their quality of life. The aim of this case series is to describe the ocular manifestations of GVHD patients. This was a retrospective, observational case series investigating the ocular complications of GVHD. The medical records of all patients who received HSCT in the period from October 2016 till October 2018 at King Abdulaziz Medical City in Jeddah, Saudi Arabia were reviewed. Of the 111 patients who received HSCT, 4 patients developed ocular-GVHD, and all of them had bilateral presentation. The median length of time from receiving HSCT to the diagnosis of ocular-GVHD to was 4 months. Dry eye disease was the most prevalence ocular manifestation. Outcomes varied from complete resolution with medical treatment to resistant disease. Common therapeutic modalities were used including: preservative free lubricants drops and ointment, topical cyclosporine A, topical or systemic corticosteroids and immunosuppressant agents.
