Ocular signs and risk factors of children thyroid eye disease

Abstract :

Purpose: To investigate children thyroid eye disease (TED) signs and risk factors. Patients & Methods: 67 patients with Grave's disease (GD) below 18 years were investigated. 18 of them were with thyroid eye disease signs. The median age of all patients at Graveâ??s disease diagnosis was 13.3 years. Results: The most frequent features were upper eyelid retraction (72.3%), proptosis (66.7%), and Graefe sign (44.4%). The palpebral fissure (12.9 mm) and proptosis (17.4 mm) were greater in TED group than in patients with GD without ocular signs. Our patients had no corneal pathology, strabismus or optic neuropathy. Most patients with TED had a mild activity of eye disease, and clinical activity score more than 3 points was in 16.6% of TED patients. Eyelid redness and swelling (33.3%), redness of caruncula lacrimalis (27.8%) and conjunctival injection (38.9%) and chemosis (11.1%) were presented in patients with TED. From multivariate regression analysis the risk factors which were correlated with TED development were passive and active smoking with the odds ratio (OR)=7.10(p<0.026), stressful event before GD diagnosis (OR=6.03; p<0.025), the increase of serum free thyroxine levels (OR=5.96; p<0.018) and thyroid stimulating hormone receptor antibody levels (OR=6.36; p<0.010) at GD diagnosis. Conclusion: The most common signs in children with TED were upper eyelid retraction, Graefe sign, proptosis, eyelid redness and swelling, injection of caruncula lacrimalis and conjunctivae. Patients with TED had a mild activity of eye disease. Passive and active smoking and stressful event, high serum free thyroxine and thyroid stimulating hormone receptor antibody levels at GD diagnosis were correlated with the development of children thyroid eye disease.

Jurate Jankauskiene (PhD –Medicine) now is a Professor of Eye Clinic of Lithuanian University of Health Sciences. During 1986-1994 she was a research worker of Vision Protection Laboratory of Kaunas Medical University. In 1994, she was an Associate Professor of Eye Clinic Kaunas Medical University. From 2005, she was a Professor of Eye Clinic of Kaunas Medical University (now Lithuanian University of Health Sciences). During 2006-2018, she was a President of Lithuanian Apitherapy Association. She has more than 150 publications in Lithuanian and foreign scientific journals in apitherapy and eye diseases diagnostic and treatment area. She has participated in the seminars and conferences in Holland, Canada, Ireland, Poland, Germany, England, Norway, Russia, Ukraine, Romania and Lithuania.
