Introduction: The type III open fracture of Gustillo is serious because it may not be closed and only the primary or secondary cover is the solution in order to cover the bone completely deperiosteal and in the free area, which exposes it to necrosis and to become a receiver. Material and Method: This is an open type III fracture of Gustillo and Anderson secondary to various accidents, causing considerable damage and the prognosis is not always good. Urgent care of five injured with open fractures, three of the leg and two type III femurs from Gustillo and Anderson with trimming and stabilization by OREF and secondary skin coverage. Results: Three patients out of five progressed well and the skin cover was satisfactory with bone consolidation, for the two cases an evolution towards a septic pseudarthrosis was noted Discussion: Skin coverage by locoregional total graft, free or by muscle flap are the solutions to the problems of loss of skin substance caused during open fractures that cannot be sutured initially. Conclusion: Open fractures represent a therapeutic problem whose prognosis is not always favorable and what complicates its management is the technical difficulty of achieving skin coverage.
Allache Kaci was born on December 21, 1977 in Algiers. He followed medical studies at the Faculty of Medicine of Algiers to specialize in orthographic and traumatological surgery in 2004, then he deepened to specialize in the hip, more particularly osteonecrosis and in 2019 he defended his thesis on osteonecrosis of the femoral head. And he was writing a book on osteonecrosis.
