Papilledema and abducens nerve palsy in an acquired immunodeficiency syndrome patient

Abstract :

Two cases of bilateral abducens nerve palsy with papilledema in Acquired Immunodeficiency Syndrome (AIDS) patients have been studied. However, unilateral or bilateral abducens palsy has been reported about 9% of patients with both cryptococcal infections in AIDS patients. We use the term �??divergence insufficiency�?� to describe the diagnosis and treatment of a case patient presenting with acquired disorder of ocular horizontal version, characterized by full-appearing ocular ductions and comitant esotropia at distance. We found that divergence insufficiency may be an early sign of abducens palsy and is often associated with neurologic lesions. In cases secondary to raised Intracranial Pressure (ICP), control of ICP at an early stage can prevent the extent of damage to the sixth nerve and thereby shorten recovery time.
