Per year we are treating 80-90 spinal cord injury, 40 % of their with neurological deficits. Methods: We present you our experience about the spinal cord injury and focused the second damage to the spinal cord caused of stretching from the adhesive arachnoiditis. Results: We describe how it?s matters to reduce the arachnoiditis in spinal cord injury with the intramedullary contusion and edema because this fact worse the recovery of the spinal cord and cause the post-traumatic syringomyelia. Conclusion: The arachnoiditis is a physiological condition after the secondary tissue damage in spinal cord injury for the release of the inflammation agents and cause leptomeningeal adhesion between the arachnoid and the dura with stretching and compression of the spinal cord. It matters to avoid this complication with the dura opening and washing with saline solution the arachnoid and performing a dura plastic
