Leukaemia is defined as neoplastic proliferation arising from hematopoietic and progenitor cells. Acute myeloid leukaemia (AML) is the most prevalent type of adults leukaemia. According to a recent census by the Saudi Cancer Registry (SRC), higher incidences of AML were reported in females with poor prognosis. The involvement of the oral cavity is part of widespread disease; however, it can be the first manifestation of the disease. Petechiae ecchymosis, gingival bleeding with painless hyperplasia, ulcerative necrotic lesions, and pale oral mucosa are the most common reported lesions. Case description A 49 Y/O Saudi female patient reported to Oral Medicine Clinic/ PSMMC with a chief complaint of severe oral ulcerations. The patient was dehydrated, unable to talk or swallow. Intraoral examination revealed severe ulceration involving dorsum, lateral, and ventral surfaces of the tongue with crusting of the tongue tip. The preliminary diagnosis was vesiculobullous lesion. Patient was admitted for IV fluid replacement and intravenous steroid. Initial CBC showed a moderate increase in WBC. On the third day, tongue ulcers almost disappeared, but WBC count was increased gradually on a daily basis until it reached a critical value of 100 per microliter of blood on the eighth day. A haematologist was consulted, bone marrow biopsy was carried out, it revealed acute myeloid leukaemia. Conclusion The aim of this report is to present an unusual initial oral manifestation of leukaemia that mimics vesiculobullous lesions.
