This presentation is designed to provide insights on the application of conventional and advanced cardiac ultrasound concepts used in the assessment of myocardial deformation in congenital heart disease and palliative procedures. The presentation includes the maturation of myocardial mechanics, fiber orientation, contraction, relaxation, twist, torsion. An introduction to speckle tracking echocardiography-strain imaging including definitions and validation in the paediatric population will be discussed. The presentation will conclude with the clinical application of strain imaging in complex case examples including, tricuspid atresia, chemotherapyinduced cardio toxicity, graft rejection, and hypoplastic left heart syndrome. Learning Objectives: ΓΆΒ?ΒΆ Describe the basics of myocardial fiber architecture ΓΆΒ?ΒΆ Explain the three principle vectors of myocardial deformation ΓΆΒ?ΒΆ Explain basic concepts of speckle tracking echocardiography ΓΆΒ?ΒΆ Interpret normal from abnormal strain waveforms ΓΆΒ?ΒΆ Review cases commonly encountered in paediatric clinical practice
