Introduction: Medical records are important for adequate continuity of care in patients’ management. Good medical records are essential for health professionals to use when defending against complaints or claims of clinical negligence because they give insight into the clinical judgement that was being used at the time. Records that are sufficient for continuity of treatment are typically also sufficiently thorough for legal purpose. Methods Retrospective audit Discussion. In our observations, we found that in the initial audit, •Areas requiring improvement •Elective/Emergency •Operative Diagnosis/Indication •Details of closure technique •Signing of the post operative note Conclusions Our study showed significant improvement in the quality of operative notes after the implementation of recommendations following the initial audit of operative notes in Glan Clwyd Hospital. Such audits should be conducted routinely in all surgical departments to maintain and improve standards of documentation of operative notes
